Becoming a New Patient


Thank you for your interest in becoming a patient with ADI Health. We are striving daily to meet the needs of as many patients as we can, but the demand for our services is high.

We have outlined the new patient process for your convenience and request that you start the process by filling out the NEW PATIENT REQUEST FORM at the bottom of this page.


Step 1: Complete the New Patient Request Form (Below)

Step 2: Scheduling Your New Patient Appointment(s)

Once we review and approve your information, someone from our office will call to schedule your appointment. If you don’t receive a call in 3-5 business days, please call 847-577-7705.

If you are being scheduled to see a psychiatrist, you will be scheduled for two appointments (60m for the initial evaluation and 30m in 2 weeks for the follow up visit for all psychiatry and medication management visits). Once these appointments are made, you will receive links for your telemedicine visits (if applicable) from reminders@drchrono.com.

*Please make sure you add reminders@drchrono.com to your safe senders list (check spam the first time if you do not see it in your inbox) — all telemedicine confirmation and appointment reminder emails will come from this email address.

Step 3: Accept OnPatient Invite & Use Browser to Create Account (48 Hour Window; OnPatient App does not work)

You will also receive a time-sensitive invitation to register to OnPatient. You will have a limited time-frame to register (48 HOURS), so please open the email from “onpatient Invite” immediately and follow the directions outlined in The OnPatient Registration Process Guide to create your account and connect to Atula Sharma, MD or Morgan Fallor, MD. All patients seeing Dave Hansen, LCPC MA, Kate Cicchelli, EdD, or Jailan Hanafy, MD will be assigned to Atula Sharma, MD as their provider.

*If you are seeing a psychiatrist that is new to our team, Dr. Sharma or Dr. Fallor will be assigned as your primary provider, and she will work collaboratively in shared medical decision making to ensure that you receive the best care possible.

Step 4: “Check-in” via OnPatient

Once you are logged in to OnPatient, go to the Appointments tab and follow the directions in the Registration Patient Check-in & Onboarding Guide to complete your profile, upload images of your insurance card (front and back of your card), submit your medical history (under reason for visit select new patient form), and review & sign off all policies and consent forms.

**WARNING - If we do not receive all required documents within 48 hours of scheduling your appointment, your appointment may be cancelled and released to another patient on our waitlist.

ONPATIENT WON’T LET YOU COMPLETE YOUR CHECK IN? MAKE SURE YOU’VE COMPLETED ALL REQUIRED SECTIONS AND SAVED YOUR SIGNATURES

Please note that all required fields must be filled out to check-in. If you have issues uploading the copies of your card, please send them as an attachment via OnPatient Messages (in PDF, JPEG, or PNG file format; 1 JPEG per message) or by replying all to this email. And if you have any issues with completing the check-in, please email jamie@adi.health or call 847-577-7705 and our Front Office Manager, Jamie, will set up a call to guide you through the process.

IF YOU CANNOT LOGIN TO ONPATIENT,

  1. Clear your cache and/or try using an incognito or private window

  2. Call Jamie at 847-577-7705 to have your account reset

  3. If none of the above work, please submit the following for via fax (847-929-9138) or email (jamie@adi.health): Welcome Form (front & back, plus two signatures)

Step 5: Submit Remaining New Patient Forms via OnPatient Messages

Submit the below forms (ALL REQUIRED) via OnPatient Messages (PDF, JPEG, PNG only; 1 attachment per message only), fax, or email:

  1. An Authorization for Use or Disclosure of Information for Patient Care (ROI) should be completed for any/all parties that you would like to authorize our team to connect with regarding your care. This could include a family member, therapist, or provider from whom we should connect with regarding your medical records (REQUIRED FOR ALL PATIENTS WHO CURRENTLY HAVE A PSYCHIATRIST OR THERAPIST, AND FOR ALL PATIENTS OVER THE AGE OF 18 WHO WOULD LIKE THEIR PARENTS/ANOTHER PARTY TO BE INVOLVED IN THEIR CARE)

  2. Credit Card on File Agreement

  3. Parent Questionnaire (REQUIRED ONLY FOR PATIENTS UNDER THE AGE OF 19)

  4. Insurance Card (front & back)

  5. Drivers License (front & back - for the patient OR responsible party)Any additional information you’d like to share with the provider

Prior to your First Appointment

Timeline: Complete within 48 hours of scheduling your appointment

  • To gain a better understanding of your behavioral health insurance coverage (including if telemedicine visits are covered) and what your out-of-pocket costs might be, please call the number on the back of your insurance card and connect with a representative at your insurance company.

  • In addition to inquiring about your coverage, please confirm the mailing address for your behavioral health claims. If this address and company differs from your primary coverage information, please email this information immediately. If your mental health carrier is not BCBS PPO, Aetna PPO, or Cigna PPO, you will be responsible for all payments.

The Day of your Appointment

Timeline: 5-10 minutes prior to your first appointment

Five to ten minutes before your appointment, please open an incognito window (Chrome) or private window (Safari) and click the link in the email reminder to join the session. The Telemedicine Appointment Guide offers more information on this and an alternate way to join your session via OnPatient (you will need your OnPatient login information to connect successfully).

If you leave the session unattended, this may create unwanted technical issues and you may need to either refresh your window or close the session and click on the link to join again.


IMPORTANT (SECOND APPOINTMENT ONWARD):
You will be required to check-in to all follow-up visits (second appointment onward) to verify your information and submit your “Reason for Visit” form via OnPatient at least 24 hours prior to your visit.

Following your First Appointment

Timeline: Within 24 hours of completing your first appointment

Please make sure you have the link to your next appointment -- if not, please send us a message to your care team via OnPatient immediately.

  • If you are being prescribed a medication (this may be discussed during your appointment, is dependent on a variety of factors/the diagnosis, and is not guaranteed), you will need to complete a Medication Consent form and return it to the office via OnPatient Messages (in PDF, JPEG, or PNG file format). before it can be called in/submitted electronically to your pharmacy.

Prior to your Second Appointment (via Email & OnPatient)

Timeline: At least 48 hours prior to your scheduled appointment

  • Send us any lab work, forms, and/or information requested by Dr. Lachin at your initial appointment via OnPatient Messages as an attachment (PDF, JPEG, or PNG only). If you need to send a document using a different format, please email me directly.

  • You will receive a reminder email 24 and 48 hours prior to your appointment. Check-in at least 24-48 hours before your appointment by completing all necessary fields in the demographic information (such as allergies, medications, etc.) and completing the brief returning patient assessment called “Reason for Visit” (you can now skip the new patient form). Here is the Existing Patient Check-in Guide, for additional guidance. At this time, please review your patient statements and/or make any copayments to your account via OnPatient or by calling 847-577-7705. You can also send us a message authorizing us to charge the card listed on file / on your credit card authorization form.

  • You will use the link in the reminder email to join your appointment 5-10 minutes before the appointment starts. **Please do not forget to check-in on OnPatient to complete your reason for visit form prior to joining your appointment**

Following your Second Appointment

Timeline: Within 24 hours of completing your appointment

After your second visit, if we do not connect with you within 24 hours of your appointment, please connect with your care team via OnPatient Messages, by connecting to our Front Desk at 847-577-7705.

Our schedule fills up very quickly, please make sure your follow up appointment is scheduled ASAP. If not, you may experience delays in your refills and or treatment plan, which we would like to avoid at all possible costs. Thank you!

In Case of Emergency

Please call 911. If you are hospitalized or require urgent assistance / care during non-business hours, Dr. Sharma can be reached via her answering service at 800-330-1275. All non-urgent calls will be billed a minimum of $25.
